CANCELLATION & RESCHEDULE POLICY; TERMS & POLICIES AGREEMENT

CANCELLATION POLICY

• Pre-paid deposit fees are NON-REFUNDABLE. There are zero exceptions. Your remaining balance will be due on the day of your appointment.
• We understand that life happens, but please be advised that we will only allow a maximum of 10 MINUTES of “running late” before marking you as a “no-show” & forfeiting both your appointment and your non-refundable deposit.
• Should you decide to cancel your appointment for any reason, you will not receive your pre-paid non-refundable deposit back.

RESCHEDULE POLICY

• ONE reschedule request is allowed without penalty for a booked appointment if and only if you submit your reschedule request through the online booking system more than 24 hours in advance. Additional reschedule requests for the same appointment WILL NOT BE HONORED & will result in penalty of both your appointment and your pre-paid non-refundable deposit being forfeited. You will need to book a new appointment and pay a new non-refundable deposit to get back on the calendar should you attempt to reschedule your booked appointment more than once.
• To submit your penalty-free, one-time reschedule request, view your appointment details in your acceptance text and/or email & find the reschedule button to choose a new day and time.
• Text messages or emails sent to Katrina verbalizing a reschedule request WILL NOT BE HONORED. All reschedule requests must be completed through the online booking system before the 24-hour cutoff time.

TERMS & POLICIES AGREEMENT

• As a condition of booking an appointment with Brows By Katrina LLC, photos & or videos of my eyebrows will be taken before/during/after my procedure for insurance purposes. These photos & or videos may also be used for Brows By Katrina LLC's online portfolio, website, social platforms, & or online educational demonstrations. A photography/videography release consent form will be sent to me for signature. If I do not feel comfortable showing my face, a mask can be provided to me upon request.
• I understand that if I am more than 10 minutes late, my appointment and my pre-paid non-refundable deposit will be forfeited immediately.
• I understand that if I cancel this appointment for any reason, my pre-paid non-refundable deposit fee will not be refunded.
• I understand that I can reschedule this appointment without penalty ONE TIME ONLY, so long as my appointment is more than 24 hours away. I understand that if I attempt to reschedule this appointment a second time, that my request will be denied resulting in both my appointment and my pre-paid non-refundable deposit being forfeited.
• ACNE: I understand that Katrina can NOT tattoo over acne as a safety precaution. I understand that it will be my responsibility to thoroughly check my eyebrows for acne on the day of my appointment & that if I find acne is present in/around my eyebrows on the day of my appointment, that I must text photos of the acne in my eyebrows to Katrina for review & next steps. I understand that if I show up to my appointment with acne in/around my eyebrows, I will be sent away; my appointment and pre-paid non-refundable deposit fee will be forfeited; & that I will need to book a brand new appointment & submit a brand new non-refundable deposit to get back on the calendar.
• I have read and I understand Brows By Katrina LLC’s Cancellation Policy which includes both Cancellation & Reschedule policies.
• I understand that I must read and follow all of the pre-care instructions that will be sent to me once my appointment is accepted.
• If I have had previous tattooing/permanent makeup done on my eyebrows by another artist, I have received written approval to book this appointment from Katrina via email.
• I understand that if I do not attend the required touchup appointments within the required touchup timeframes (6-8 weeks from my initial appointment, and every 6-24 months thereafter for maintenance), that I will be reset to a new client and will incur the new client fee. I understand that there are zero exceptions to these rules.
• I understand that all appointments are reserved with non-refundable deposit fees at the time of scheduling.
• I understand that legally recognized service animals may accompany me to my appointment and that emotional support animals and/or pets are NOT allowed to accompany me to my appointment.
• I understand that I may bring a guest with me to my appointment but that they are not allowed in the procedure area and must wait in the waiting room.
• I am 18 years of age or older, & will present my ID to prove as such. I understand that no persons under the age of 18 are allowed in the procedure room.
• I understand that Brows By Katrina LLC does NOT perform cosmetic tattooing on pregnant or nursing mothers, or any persons fighting life-threatening illness.
• I understand that if I am sick or have any onset symptoms of illness, that I should reschedule or cancel my appointment.
• I understand that I will be given aftercare instructions to follow for infection prevention and optimal healing upon completion of my appointment, and that these instructions are also available online at browsbykatrina.com
• I understand that results will vary per individual and that there are no guarantees as to how well the cosmetic tattoo will take on my skin in particular.
• I understand that by booking an appointment, I am agreeing to Brows By Katrina LLC's Cancellation Policy, Reschedule Policy, & Terms & Policies agreement.
